I know that experience firsthand. I've had six surgeries — both shoulders rebuilt, my left Achilles reattached, prostate surgery, and two mechanical discs placed in my cervical spine. Chronic pain has been part of my life for a long time. And when the migraines started in 2016 — after that neck surgery — I found myself deep in research, trying to understand not just what was happening, but why. Why does the pain persist when nothing shows up on the tests? Why does stress make everything worse? Why does the body seem to stay in a state of alarm even when the original threat is long gone?
The answer I kept coming back to was the nervous system. And once I understood that, everything else started to make more sense.
You're not imagining it. And you're not broken.
What's happening is something more specific — and more addressable — than most people realize. Your nervous system got stuck. And a nervous system that's stuck in survival mode will generate real, measurable symptoms: pain, fatigue, hypersensitivity, anxiety, and a profound sense that your body is working against you.
Understanding this doesn't fix everything. But it changes the question from what's wrong with me to what does my nervous system need — and that's a question we can actually work with.
The Two Modes of Your Nervous System
Most people have heard the terms sympathetic and parasympathetic. Fight-or-flight versus rest-and-digest. But knowing the labels and understanding what it actually feels like to live in one mode chronically are very different things.
The sympathetic nervous system is your activation system. It's designed for short bursts — a threat appears, the system mobilizes, you respond, the threat passes, and the system returns to baseline. Heart rate elevates. Muscles tense. Breathing becomes shallow and fast. Digestion slows. Pain sensitivity increases. All of this is appropriate and adaptive when the threat is real and temporary.
The parasympathetic nervous system is your restoration system. When it's running, heart rate slows, muscles release, digestion resumes, and the body shifts its resources toward repair and recovery. This is the state where healing happens. Where sleep is restorative. Where pain signals are processed rather than amplified.
The problem isn't that the sympathetic system exists. The problem is when it stops turning off.
When you live in chronic sympathetic activation, your body is essentially running a continuous low-grade emergency. Everything costs more energy than it should. Sensations that would be minor in a regulated nervous system get amplified into significant pain. Sleep doesn't restore you because the system never fully downshifts. And the longer this goes on, the more the nervous system learns that high alert is the default — not the exception.
How the Nervous System Gets Stuck
There's a concept in stress physiology called allostatic load — the cumulative wear on the body from sustained stress and adaptation. Think of it as the total weight your nervous system has been carrying over time.
Illness. Injury. Trauma. Grief. Chronic pain itself. Years of pushing through when the body was asking for rest. Each of these adds to the load. And at some point, the nervous system — which is fundamentally a pattern-recognition and threat-response system — concludes that the world is not safe. That relaxing is dangerous. That it needs to stay on.
This is not a character flaw. It's not weakness. It's a nervous system doing exactly what it was designed to do: protect you. It learned, through experience, that staying alert kept you functional. The problem is that the lesson outlasted the circumstances that taught it.
For many people with fibromyalgia, the central sensitization that drives their pain is rooted in exactly this mechanism. The nervous system has become so sensitized to threat signals that it begins generating pain in the absence of tissue damage. The pain is real — it's just being produced by a system that's stuck in a pattern, not by ongoing injury.
What Living in a Stuck Nervous System Actually Feels Like
I want to be honest about this part, because I think it gets glossed over in a lot of wellness content.
Living with a chronically activated nervous system is exhausting in a way that's hard to describe to someone who hasn't experienced it. It's not just the pain. It's the hypervigilance — the sense that your body is always braced for something. It's waking up tired even after a full night's sleep. It's the way a stressful conversation can send your symptoms spiking for hours afterward. It's the frustration of knowing, intellectually, that you should relax — and having your body completely ignore that instruction.
I spent years in that place. And the thing that started to shift it, more than anything else I tried, was learning to work directly with my nervous system through breath. Not because breathing is magic, but because it's the one autonomic function you can consciously control — and changing your breath changes everything downstream.
The Three Signals Your Nervous System Needs
Nervous system regulation isn't complicated, but it does require consistency. Based on my own experience and everything I've read and tried, there are three things a dysregulated nervous system needs most:
1. Safety
The nervous system responds to signals of safety the way a frightened animal responds to a calm voice. Slow breath is a safety signal. A soft, unhurried exhale tells the vagus nerve: the threat has passed. You can stand down. Gentle imagery — picturing a place that feels genuinely calm and real to you — adds another layer. The nervous system doesn't fully distinguish between a real safe place and a vividly imagined one. Use that.
2. Rhythm
Consistent, rhythmic breathing is one of the most powerful regulators of the autonomic nervous system. The reason isn't mystical — it's mechanical. Rhythmic breathing creates rhythmic variation in heart rate (what's called heart rate variability), and high heart rate variability is a direct marker of a well-regulated nervous system. The more consistently you practice, the more the autonomic system learns that rhythm is available. Over time, it begins to default toward it.
3. Time
This is the one people most often underestimate. Nervous system regulation is not a one-session fix. It's a practice — in the same way that physical therapy for an injury is a practice. You're not just relaxing in the moment. You're gradually teaching a pattern-recognition system to recognize a new pattern: that rest is safe, that the threat has passed, that it's okay to come down.
Some people feel meaningful shifts within a few weeks of consistent practice. For others, it takes longer. The nervous system learned its current patterns over years. It doesn't unlearn them overnight. But it does unlearn them — and that's worth holding onto.
What This Looks Like in Practice
You don't need an hour a day. You need consistency more than duration.
A simple starting framework:
Morning (10 minutes): Before you check your phone or engage with the day, do a guided breathing session. This sets the autonomic tone for the hours ahead. Even five minutes of slow, rhythmic breathing with a longer exhale than inhale is enough to shift your baseline.
During symptom spikes (5 minutes): When pain flares, anxiety rises, or you feel the system tightening — stop. Breathe. Use the reset breath: inhale four counts through the nose, exhale six to eight counts through the mouth. Ten repetitions. This won't always eliminate the symptom, but it interrupts the amplification cycle.
