Fibromyalgia is one of the most misunderstood conditions I've come across in all my years of living with chronic pain.
I've dealt with chronic pain for most of my life. And when migraines entered the picture in 2016, I went deep into research — reading everything I could find about pain, the nervous system, and why some bodies seem to stay stuck in a state of suffering even when the tests show nothing wrong.
Fibromyalgia kept coming up. Not because I have a formal diagnosis, but because so much of what I read about it described experiences I recognized: pain that doesn't have a clear structural cause, fatigue that sleep doesn't fix, a nervous system that seems permanently set to high alert.
What I want to say first, before anything else, is this: the pain is real. The fatigue is real. The cognitive fog, the hypersensitivity to sound and light and touch — all of it is real. It's just not being caused by what most people assume.
Fibromyalgia is fundamentally a nervous system condition. Specifically, it involves a process called central sensitization — where the central nervous system becomes so amplified in its threat response that it begins generating pain signals in the absence of tissue damage. The volume on the pain dial gets turned up and, for reasons that aren't fully understood, it doesn't turn back down.
Understanding this matters because it changes what you reach for. And breathing — specifically slow, guided, rhythmic breathing — is one of the most direct ways to turn that volume down.
Why Fibromyalgia and the Nervous System Are Inseparable
To understand why breathing helps, you first need to understand what's happening in a fibromyalgia-affected nervous system.
Under normal circumstances, the brain processes pain signals and modulates them — amplifying some, dampening others, based on context and perceived threat level. A slow, extended exhale — longer than the inhale — activates the parasympathetic nervous system through the vagus nerve. Heart rate slows and becomes more variable — a sign of a healthier, more regulated nervous system. Muscle tension decreases. Cortisol and adrenaline levels begin to drop. And critically, the pain-processing system begins to downregulate — the volume on the pain dial starts to come down.

- Close your eyes if that's comfortable, or soften your gaze downward.
- Inhale slowly through your nose for a count of four.
- Exhale slowly through your mouth for a count of six to eight. The exhale should be gentle — not forced, not rushed. Think of it as a slow, quiet release.
- Pause briefly at the bottom of the exhale before the next inhale.
- Repeat for ten breaths, then rest and notice what you feel.
The pause at the bottom of the exhale is something I've found particularly useful. It seems to give the nervous system a moment to register the signal before the next activation cycle begins.
What you're looking for: a slight softening in the body. Shoulders that were up near your ears beginning to drop. A jaw that was clenched beginning to release. A sense — even a small one — that the system is beginning to slow down.
Consistency Matters More Than Duration
One of the most common mistakes I made early on was treating breathing practice as something to do only when the pain was unbearable. That's like only doing physical therapy when you're in crisis — it helps in the moment, but it doesn't build the underlying capacity you need.
The real benefit of breathing practice for fibromyalgia comes from consistency. When you practice slow, rhythmic breathing regularly — even for just ten minutes a day — you're gradually rerecovery journey the autonomic nervous system. You're teaching it that the parasympathetic state is available, that it's safe to go there, and that it can return there reliably.

Here's what I use as a starting point:
Morning (before you get out of bed): Five to ten minutes of slow breathing before you engage with the day. This is particularly important for fibromyalgia because the morning is often when the nervous system is most activated — the transition from sleep to waking can trigger a sympathetic response. Starting with breath gives you a different entry point into the day.
During flares: When pain spikes, stop what you're doing and do ten reset breaths. You're not trying to eliminate the pain — you're trying to interrupt the amplification cycle. Even a small reduction in sympathetic activation can make the flare more manageable.
Evening (before sleep): A longer session — fifteen to twenty minutes — helps the nervous system downshift enough for restorative sleep. This is one of the highest-leverage practices for fibromyalgia, because poor sleep is both a symptom and a driver of central sensitization.
